Red Flags and the Rescue Call

Failure to rescue is not a patient whose collapse was unpredictable — it is a patient whose warning signs were written down and not acted on. Triage ten real sets of findings into monitor, notify or rapid response; then build an SBAR call and have it scored on completeness; then climb the chain of command when the first call does not work.
