The Charting Bench

An intervention with no documented reassessment is indistinguishable, in a chart and in court, from one that never worked. Rebuild four vague entries into notes a nurse who has never met the patient could act on — then set the right reassessment window and decide whether each outcome was met, partially met or not met.
Pick an entry to rebuild
What was actually charted
Select every line that belongs in the note
Your note as it stands
Select lines on the left and the entry assembles here in chart order.
Nothing charted yet
Documentation must be timely, factual, complete, organized and objective. Chart what you observed and what you did — never what you concluded about the patient’s character.
The test for any entry. Could a nurse who has never met this patient reconstruct what happened and decide what to do next from your words alone? If not, add measurements, times and the response to your intervention. Never chart in advance, never chart for someone else, never delete an entry — correct an error by facility policy for an addendum, with the reason and your initials, and label a late entry with both the time of the event and the time of the entry.
Implementation is assessment too
Assess is the plan still right? Intervene monitor during Reassess and chart the response evaluation feeds straight back into assessment THE ORDER IS NEVER “DO THE TASK, THEN LOOK AT THE PATIENT”
