Rewrite the Chart: Turning Labels into Observations

The note on the right is real in the sense that every phrase in it gets written every day. Each struck-through phrase is an interpretation of motive or a piece of moral vocabulary pretending to be an assessment finding. Work through all six: pick the replacement that is observable, reproducible and useful, and watch the note turn into something a colleague can actually act on.

Choose the replacement

Select a flagged phrase to see four possible rewrites. Only one of them describes behaviour instead of judging it.

The note, live

Struck-through text is what was written. Green text is what you have replaced it with. Every one of these labels travels through the chart and changes how the next clinician listens.

Why it matters

Language is the intervention you control completelyDescribe the behaviour, describe its context, and describe what it may be communicating. Interpretive labels are not observable — they are your inference about somebody else's motive, and once written they are read as fact.
