Where Did the Message Die?

Hearing happens to you; listening is six separate jobs, and each one can fail on its own. Take one real nursing handoff and break or repair each stage yourself. Watch which of the seven facts actually survive into the shift — and what happens at 02:10 when one of them didn’t.
The message, sent once, at normal speed
Outgoing nurse · 15 seconds“Bed 4, Mr. Ellison, seventy-two, came in with chest pressure, troponin negative times two, he’s on telemetry, pain-free since about four, and he gets really anxious when the alarm goes off, so warn him before you silence it.”
The six stages — tap to break or repair
Why this matters. “You never told me” and “I told you” are usually both true — about different stages. The repair for a broken attention stage is nothing like the repair for a broken memory stage, which is why “try to listen better” fixes neither.
