A handoff is a paragraph you compose under time pressure and then have to prove landed. Pick the structure the situation actually calls for, drop each line into its element, and leave out the ones that burn your listener's attention. The person on the other end will answer you based on what you said — and on what you didn't.
1 · The situation
2 · Which structure?
3 · Place every line — or leave it out
The handoff as delivered
Read it out loud. That is the actual test.

“Watch her pressures” is not a plan. Convert your worry into something the next clinician can execute at 0300 without paging anyone.
The handoff is not finished when you stop talking. It is finished when the receiver says the plan back and asks at least one question. Silence after “any questions?” should feel slightly uncomfortable before you move on.
